Add your name to the form, then send it back to the front desk. Data entered here is stored only on your device and never transmitted or stored by a server.
Your drawing will be erased when the page is refreshed or closed and after you click the Download Drawing button.
Mark the areas on your body where you feel your symptoms. Include all affected areas. You can select from any of the symbols below:
To mark areas of radiation, you can select "free draw" to create an arrow from where it starts to where it stops. Please extend the arrow as far as the pain travels.
